The Surgeon Satisfaction Survey Questionnaire

The Surgeon Satisfaction Survey is an objective way to find out how they view your OR.

It asks specific questions about:

• Anesthesia Services
• Equipment and Supplies
• Medical Advisory/OR Committee
• Quality Improvement
• Overall Impression

Open-ended questions encourage your surgeons to comment on how your facility compares with other facilities and to identify areas they feel may need improvement.

What You Will Need to Do

The Surgeon Satisfaction questionnaire is simple and easy for your surgeons to fill out. It will take them only a few minutes to respond. You’ll want to send the survey to all your surgeons.

You will need to provide the names and addresses of your physicians that you want surveyed, and we’ll ask you to sign a brief cover letter encouraging their participation.

The questionnaires will be mailed to the physicians from our research company. The responses are returned by mail in a post-paid envelope or faxed directly to the research firm.

What We Do

You will receive a full report of the responses to the survey questions and the open-ended comments. The surgeons are only identified by a code number.
